India is the world's most established medical tourism destination: 7.3 million international arrivals in 2024, an $18 billion industry, and a private hospital network of unrivalled scale — offering 60–90% cost savings on procedures ranging from cardiac bypass to neurosurgery, backed by more JCI-accredited facilities than almost any country outside the United States.

India does not merely compete for the medical tourist's attention — it defines the category. With 7.3 million international patient arrivals in 2024 generating an estimated $18 billion, the country's private healthcare system has been purpose-built, over three decades, to serve people who travel for treatment. The infrastructure is staggering: Apollo Hospitals alone handles more than 200,000 international patients annually across 60+ hospitals; Medanta Gurugram, repeatedly named the best private hospital in India by Newsweek, sees over 20,000 international patients a year; Fortis, Max Healthcare, Narayana Health, and Kokilaben Ambani Hospital round out a tier of providers that would not look out of place in Singapore or Germany — at a fraction of the price.

Where India leads the world

Our research scores India at the top of the range — 5 out of 5 — for overall surgical cost savings, cancer treatment cost savings, and hospital accreditation. Those three pillars sit under the same roof: 45 JCI-accredited hospitals (among the highest concentrations globally) and 1,299 NABH-accredited facilities maintain the quality baseline against which the cost differential is measured. Cardiac bypass surgery at a leading Apollo or Narayana centre costs $5,000–7,000, against $100,000 in the United States; a complete cancer treatment package runs $3,000–30,000, versus figures that routinely exceed $150,000 in the West. Immunotherapy is available at 70–80% savings; CAR-T cell therapy, one of the most expensive oncology interventions anywhere, at up to 90% savings.

Cardiac surgery is the signature speciality (score: 4.5). India performs more than 140,000 open-heart procedures annually across 175+ dedicated cardiac centres. Leading institutions — Asian Heart Institute, Apollo Heart Institute — report bypass success rates of 99.6–99.83%, matching best-in-class Western benchmarks. The national cardiac surgery mortality rate of 2.40% keeps India just below the very top tier (the Score 5 threshold is under 2%), and paediatric cardiac outcomes remain a documented weakness, but for adult cardiac work the combination of volume, technology, and price is unmatched.

Organ transplantation scores 4.5. India performed 1,851 transplants on foreign nationals in 2023, with kidney wait times of six to eight weeks for living-donor procedures, one-year kidney survival rates above 95%, and costs of $13,000–24,000 compared with roughly $442,500 in the United States. Neurosurgery scores the same: India has 2,954 trained neurosurgeons (the highest annual training output globally), five Gamma Knife centres, seven CyberKnife centres, and five facilities with intraoperative MRI.

Ayurveda and traditional medicine deserve separate mention. India scores a perfect 5 for Ayurveda quality — 3,885 AYUSH hospitals, 932 educational institutions, and a federal Ministry of AYUSH overseeing standards. For patients seeking evidence-informed traditional medicine alongside modern intervention, this is a depth of provision no other country approaches.

What patients should weigh

India's great strength — sheer scale — creates its chief challenge. Quality varies enormously between an Apollo quaternary-referral centre in Chennai and a private hospital in a Tier 2 city. Our research identifies consistent geographic concentration: the best services cluster in southern states (Tamil Nadu, Telangana, Karnataka) and in Delhi-NCR, with the middle of the country underserved. Diagnostic accreditation scores 3.5, reflecting patchy NABL certification outside the flagship institutions. Cosmetic surgery — particularly breast and buttock procedures — scores lower (2.5 and 2.0 respectively), driven by thinner regulatory oversight of aesthetic clinics (score: 3.0) and credential inconsistency among cosmetic surgeons (score: 3.5).

Certain categories are effectively closed. Surrogacy is prohibited by law for all foreign nationals under the Surrogacy (Regulation) Act, 2021. Assisted dying is unavailable in any form. Psychedelic-assisted therapy (psilocybin, ibogaine, ayahuasca) is illegal. Palliative care and end-of-life counselling are underdeveloped (scores of 2.0–3.0), and medication availability for some specialised drugs scores 2.0, with supply-chain gaps for cutting-edge oncology agents.

The practical proposition

The government's Heal in India initiative has streamlined the medical visa process and built a centralised cost-comparison platform — a material improvement for patients navigating an unfamiliar system. English is the clinical working language across every major private hospital, removing the language barrier that complicates care in Thailand, Turkey, or Mexico. International insurance is broadly accepted at accredited centres, and zero-interest EMI financing is widely available.

India is the right destination for patients whose priority is established, high-volume surgical care — cardiac procedures, major oncology, organ transplantation, neurosurgery, orthopaedics — at the lowest global price point, from providers who have spent decades refining the experience of treating international patients. It is not yet the destination for cosmetic surgery, progressive palliative care, or any treatment that depends on uniformly high standards across the whole system rather than excellence at a handful of flagship names.
